When should I arrive for my appointment?
On our website click on “PATIENT FORMS” tab print out all appropriate forms. Read all forms carefully, fill out all forms and sign them. Bring all completed forms with you this will save you a lot of in waiting room. If you are being referred by another doctor, Please bring referral slip all records, insurance information and all lab and MRI reports with you. If you don’t have reports or films please bring the name and contact number of the place where it was done. If you are not being referred please bring your insurance information and any MRI’s- X-rays of the affected area. You should arrive 15 minutes before your appointment to complete paper work.
